Liability Waiver Instructions

 
 

The Front requires that all day guests, event participants, and members have a signed liability waiver on file at The Front. 

If the climber is under 18 years of age, a parent or legal guardian must sign the waiver.  If the parent or legal guardian will not accompany the minor to The Front, a signed copy of this waiver must be sent with the child. 

You have three choices for completing the waiver.  Choose one, then click 'continue':
1) complete the waiver in person at The Front (do nothing now).
2) print a blank waiver on your printer; fill in, sign and bring/mail to The Front. (please disable pop up blockers)
3) complete the on-line waiver.  Fill in the following form.  In the next step, you will be presented with a completed waiver, ready for your signature/acceptance.  Once accepted, you will be emailed a copy of this document and a link to activate the waiver.
